Assesment Programme

Performance Physio Ltd is more than happy to offer a rewards programme to any large organization that believes it can supply our pre-screening assessment forms to their members.

In order to take part in this programme you simply need to send/email us a copy of your organizations logo. We will then print order forms with your organizations logo on it and will pay a commission on each sale made via any order made using your brand specific order form. We are happy to develop additional or omit certain questions on the form as you see fit for your membership.

Note: Logos should be a maximum of 50 pixels high and 100-150 pixels wide.

Branded Signage

The image to the right shows an example of Branded Signage for your organisation or clinic. These perspex or foam core signs can carry your membership logo and can be fixed to most surfaces using 4 stainless steel wall mounting bolts. Prices are based on volume and start at £50 ranging up to £200 for one off designs.
